The British royal, who gave birth to her second child, Princess Charlotte, earlier this month (May15), has teamed up with the Together for Short Lives charity to raise awareness of the 49,000 children living in Britain with life-threatening illnesses.

The duchess writes, "For families of children with life-limiting conditions, every moment is precious and every memory needs to be cherished and celebrated... Children's hospices strive to create happy moments in the most difficult times any family could face... I hope you will join me in supporting Children's Hospice Week and work to make every moment count for all of our families."

The Duchess and her husband Prince William welcomed their second child on 2 May (15), and they have since relocated to their country estate in Norfolk, England for the summer (15) with Princess Charlotte and their son Prince George.
