The 40-year-old star arrived in a vintage Rolls-Royce Phantom II limousine to the Marquee nightclub in New York City looking like she had aged by 40 years, but the model was in high spirits as she joked with photographers while waving around her walking stick.
The 'Project Runaway' host wore a vintage Chanel outfit and had latex skin, prosthetic effects and make-up to create the elderly appearance.
Before the transformation the blonde beauty took to Twitter and told her followers: ''Ok guys........I am going into the future (sic)''
Before adding: ''Happy Halloween. Little old me.''
Heidi had the help of award-winning make-up artists Bill Corso and Mike Marino in addition to make-up effects artist Andy Clement.
She posted multiple pictures of her getting dressed which showed off her leg veins and make-up application.
Heidi was joined by event producer Susanne Bartsch among a whole host of A-Listers to celebrate her 14th annual bash.
She has been planning her outfit since July when she posted an image of her face covered in a plaster cast on Twitter, and captioned the post: ''Started working on my Halloween costume! #HeidisHalloween. (sic)''
